| Definition. Gene therapy treats or prevents disease by changing a patient's genes — delivering a working copy of a broken gene, switching a gene off, or (newest) editing the gene in place. It is the "treat" step of the biotech toolbox. | Definicja. | How it works. The hard part is delivery: DNA cannot just be swallowed. The working gene is packed into a vector — usually a disarmed virus, which is nature's own DNA-injection device — and given two ways. In vivo: the vector is injected into the body and finds the target tissue (an eye, the liver). Ex vivo: the patient's own cells (often blood stem cells) are removed, fixed in the lab, checked, and returned. The repaired gene then makes the protein the patient was missing. | Jak działa. | Why it matters. For single-gene diseases — cystic fibrosis, hemophilia, sickle cell — drugs can only manage symptoms; gene therapy aims at the cause, potentially once and for life. After decades of setbacks it now delivers real cures, and it sets up this unit's ethics question: fixing genes in a patient's body cells is medicine — changing genes in embryos would change their descendants too. | Dlaczego to ważne. Gene therapy targets somatic (body) cells — the change is not inherited by the patient's children; that heritable version (germline editing) is a different, largely prohibited thing. Po polsku — ujęcie podręcznikowe¶
W polskich podręcznikach terapię genową definiuje się jako wprowadzanie do komórek pacjenta prawidłowej wersji genu (lub innej modyfikacji materiału genetycznego) w celu leczenia choroby. Rozróżnia się terapię komórek somatycznych — dopuszczalną i rozwijaną — od ingerencji w komórki linii płciowej, która budzi zasadnicze zastrzeżenia etyczne i jest prawnie zakazana. Jako nośniki (wektory) genów wymienia się najczęściej odpowiednio zmodyfikowane, pozbawione zjadliwości wirusy, a także liposomy. Podręczniki podają przykłady chorób-kandydatów: SCID (ciężki złożony niedobór odporności — historycznie pierwsze próby, u „dzieci z bańki”), mukowiscydozę, hemofilię, niektóre nowotwory.
Na maturze terapia genowa pojawia się w pytaniach o zastosowania inżynierii genetycznej w medycynie oraz w zadaniach na argumentację bioetyczną (np. „podaj argument za i przeciw modyfikowaniu genomu ludzkich zarodków”). Warto umieć rozróżnić trzy pojęcia, które bywają mylone: terapia genowa (leczenie pacjenta), organizm transgeniczny (trwała modyfikacja organizmu, także dziedziczna) i badania genetyczne (diagnostyka, nie leczenie).

AP Biology deep dive¶
Vector engineering is applied virology. AAV (adeno-associated virus) dominates in vivo therapy: it is non-pathogenic, largely non-integrating (persisting as episomes — great for safety, but the transgene dilutes out in dividing tissue), and its serotypes have different tissue tropisms; its ~4.7 kb cargo limit is why big genes (dystrophin, CFTR) need truncated constructs or different vectors. Lentiviral vectors integrate into the genome — ideal for ex vivo stem-cell therapy where the fix must survive endless divisions. Each choice is a gene-expression problem: promoter selection, immune visibility, and durability trade off against one another.
The safety history that shaped the field. Jesse Gelsinger's death (1999, immune reaction to an adenoviral vector) and the early SCID-X1 trials — cured immune systems, but several leukemias when the retroviral vector integrated near the LMO2 oncogene (insertional mutagenesis) — froze the field for a decade and produced today's safer designs: self-inactivating vectors, AAV's episomal persistence, and CRISPR's targeted edits instead of semi-random integration. Editing vs adding. Casgevy does not repair the sickle mutation; it disrupts the BCL11A erythroid enhancer so fetal hemoglobin (HbF) stays on, compensating for defective adult hemoglobin — an elegant regulatory workaround that shows "gene therapy" increasingly means rewiring expression, not just gene addition.
