Definition. Down syndrome is a genetic condition caused by having three copies of chromosome 21 instead of two — hence "trisomy 21", 47 chromosomes in all. Cause: almost always nondisjunction — chromosome 21 failing to separate during meiosis (most often meiosis I in the egg), so one gamete carries an extra copy. Definicja. Zespół Downa to choroba genetyczna spowodowana obecnością trzech kopii chromosomu 21 zamiast dwóch — stąd „trisomia 21”, łącznie 47 chromosomów. Przyczyna: niemal zawsze nondysjunkcja — chromosom 21 nie rozdziela się podczas mejozy (najczęściej w mejozie I w komórce jajowej), przez co gameta niesie dodatkową kopię.
How it works. A normal egg or sperm carries one chromosome 21. If the pair fails to separate, an egg with two copies meets a sperm with one — the zygote gets three, and every cell of the body inherits that extra chromosome. The extra dose of hundreds of normal genes (not a mutation in any single gene!) shifts development: characteristic facial features, some degree of intellectual disability, and more frequent heart defects. Why it matters. Down syndrome is the clearest bridge between meiosis and human life: one missed separation event, visible on one lab image, explains a whole syndrome. It anchors three ideas at once — nondisjunction, karyotype reading, and the difference between a chromosomal condition and a single-gene one. People with Down syndrome, with support, attend school, work, and live into their 60s — the biology is only part of their story. Dlaczego to ważne. Common mistake. Down syndrome is usually not inherited — the nondisjunction happens fresh in one gamete, so parents' chromosomes are normal and the recurrence risk stays low (a rare translocation form is the exception). And it is not caused by a "damaged gene": all the genes on that third chromosome 21 are normal — the problem is the extra dose. Częsty błąd. W polskich podręcznikach zespół Downa klasyfikuje się jako mutację genomową (liczbową) — aneuploidię autosomów, konkretnie trisomię chromosomu 21. Jej mechanizmem jest nierozejście się chromosomów homologicznych w anafazie I lub chromatyd siostrzanych w anafazie II mejozy; powstaje gameta z 24 chromosomami, a po zapłodnieniu zygota 2n+1 = 47. Podręczniki wymieniają zespół Downa obok innych aneuploidii: zespołu Edwardsa (trisomia 18), Patau (trisomia 13), Turnera (45,X) i Klinefeltera (47,XXY). Podkreśla się zależność ryzyka od wieku matki oraz możliwość rozpoznania na podstawie badania kariotypu, także prenatalnie (amniopunkcja, badania przesiewowe).

Na maturze najczęstsze polecenia to: rozpoznaj zespół na podstawie kariogramu, zapisz kariotyp (47,XX,+21), wyjaśnij mechanizm powstawania gamety z dodatkowym chromosomem albo porównaj mutację genomową z genową. Warto też znać poprawne, neutralne nazewnictwo — mówimy „osoba z zespołem Downa”.

AP Biology deep dive

Meiotic origin, mapped precisely. DNA-marker studies show ~90% of trisomy 21 arises in the egg, and about three-quarters of those errors trace to meiosis I — homologs failing to separate, often linked to reduced or badly placed recombination (a chiasma too near the telomere holds homologs poorly). The maternal-age effect fits meiosis's strange timeline: oocytes arrest in prophase I before birth and may wait 40+ years before completing division, with cohesion proteins deteriorating across the wait. Meiosis II errors and post-zygotic mitotic errors (causing mosaic Down syndrome, often milder) fill out the remainder.

Three cytogenetic forms. Free trisomy 21 (~95%) is the sporadic, age-related form; Robertsonian translocation (~3–4%) fuses chromosome 21 onto chromosome 14 — a balanced-carrier parent has 45 chromosomes, a normal phenotype, and a substantial recurrence risk, the one heritable form; mosaicism (~1–2%) mixes trisomic and normal cell lines. This is why "is Down syndrome inherited?" has the honest answer usually not — but check the karyotype.

Dosage, not damage. Phenotype arises from ~1.5× expression of chromosome 21's ~200+ genes; candidate contributors include DYRK1A (neurodevelopment) and APP (amyloid precursor — trisomic dosage explains the high rate of early Alzheimer-type changes). Screening has shifted to cell-free fetal DNA in maternal blood (NIPT), with karyotype or microarray as diagnostic confirmation — a tidy example of sequencing tech replacing microscopy for first-line detection.
